When you contact us
The contact page embeds a Google Form that asks for your name, email address, phone number, agency name, agency type, and an optional message. We use the information you submit to respond to your inquiry and discuss requested services. Please do not submit passwords, patient information, or sensitive incident details.
Hosting and external services
The redesigned site is served through Cloudflare. Hosting providers may process connection information such as IP addresses and request details to deliver and secure the site.
Google hosts the contact form. Video links open YouTube. Loading third-party content sends connection information to that provider. Our support portal is hosted separately by HappyFox.
Cookies and analytics
The redesigned website does not include a Google Analytics tracking script or an online checkout. Embedded third-party services may use cookies or similar technologies under their own policies. See our cookie information.
